What Is Guardian Recovery’s Case Management Program?
Guardian Recovery’s Case Management Program is an outcomes-focused case management system designed to help clients and their loved ones navigate critical transition periods including early recovery. Case Management can also provide the long-term support needed by some individuals to maintain the highest quality of life in the least restrictive environment. Each program is individually tailored to fit the unique needs of each client, providing dedicated clinical support and life skill coaching to build confidence and independence. With a proven track record of improving recovery rates, our comprehensive aftercare program creates a custom-developed path for each client, combining clinical case management with practical coaching services to guide both clients and their family members. This support is imperative through the particularly vulnerable period between inpatient treatment and fully independent living and can continue to be useful in long-term maintenance and relapse prevention.

How Our Program Works:
- Personalized Goal Setting: On the first meeting, your case manager works with you to establish goals for the next 3-6 months across all life areas: physical health, mental health, social life, recovery, work, and school/career
- 1:1 Staff to Client Ratio: Dedicated support with individualized, in-person, attention throughout your journey
- Integrated Treatment Team: Your case manager collaborates with the therapist and provides weekly updates to the entire treatment team, including family members
- Comprehensive Support: Beyond scheduled in person weekly hours, you receive unlimited talk and text access with your case manager – and your family can contact them anytime too
- Practical Life Assistance: Support with medication management (via FaceTime or in-person), doctor’s appointments, prescription refills, care provider coordination, and daily living activities
- Life Skills Development: Help with everything from budgeting and grocery shopping to meal prepping, laundry, and organizing living spaces – any life skills you need to succeed

- The transition from residential treatment to independent living can be particularly challenging for those who are new to sobriety

- Ideal for those who need help with medication management, getting to doctor’s appointments, ADLs (showering, laundry, organizing living space), and any life skills they need
Also ideal for individuals with chronic and persistent mental health disorders
- Individuals with chronic mental health diagnoses often experience frequent acute hospitalizations and repeated inpatient and residential treatment stays and this program dramatically reduces the need for hospitalization and higher levels of care
- By creating a routine that focuses on creating and maintaining a healthy lifestyle, the risk of symptom recurrence decreases
- With consistent monitoring of medication compliance and face-to-face interactions multiple times throughout the week, clients experience a reduction in decompensation and re-admission
- Instead of spending extensive amounts of time in residential treatment facilities or state hospitals, these individuals are able to experience a higher quality of life in the least restrictive environment possible

 How It Helps – “Comprehensive Support Across All Life Areas”
- Daily Living Support: Case management can assist in medication management by FaceTime or in-person observation, making sure clients get to doctor’s appointments, get refills, coordinate with other care providers
- Life Skills Development: Help with ADLs including showering, laundry, organizing living space, assist in budgeting, grocery shopping, meal prepping – any life skills they need
- Communication & Availability: In addition to the allotted in-person hours per week, clients have unlimited talk and text time with the case manager, and family can call and text the case manager anytime
- Recovery Navigation: Recovery and life coaching services help clients successfully address challenges as they arise in real time with support
- Family Integration: Family support services provided to loved ones, teaching them how to help while ensuring their own needs are being consistently met

Service Levels & Specialized Programs
- Level 1: 10 hours per weekÂ
- Level 2: 20 hours per week
- Level 3: 30 hours per week
- Level 4: 40 hours per week
- Live-in Options: 24/7 access to recovery-related support and all available treatment services
Core Services Included:
- Creation of a custom-tailored Case Management plan with specific goals evaluated and adjusted monthly
- In-person support and coaching related to the following:
- General life skills such as time management, activities of daily living, medication compliance, healthy eating, healthy lifestyle, developing a routine
- Educational and vocational development
- Personal finances and budgeting
- Family dynamics and interpersonal relationships
- Â Housing
- Â Community support meetings and general socialization
- Hobby development
In addition, the client receives:
- Long-Term Treatment Plan Management and Consultation (coordinating and supporting discharge plans, aftercare programs, and ancillary services of all types)
- Drug-screening and alcohol screening as needed (higher level confirmation and lab testing will be an additional expense)
- Support and assistance for family in person and by telephone during case management
- Detailed weekly reporting of client’s progress and ongoing challenges by phone or email
Unlimited telephone support for family and client during case management period
Additional Specialized Services:
- Sober Escort Services: Nationwide services ranging from door-to-door transfers to vacation companionship and live-in companionship services
- DBT Services: Dr. Quilter, the Executive Director, is Board Certified in DBT. Comprehensive DBT offered including:
- DBT skills training groups for self-acceptance and skill development
- Individual DBT sessions for emotional regulation and stress management
- Between-session phone coaching
- Psychological Assessment/Testing:
- Full battery including IQ and cognitive testing plus personality assessment (shows ADD, ADHD, learning disabilities, Autism, mental health disorders including personality disorders)
- Personality assessment only option for psychological diagnoses without cognitive testing
- Nutritional Services & Health Coaching: Individualized nutritional counseling focusing on specific areas requiring special attention
- Vocational Coaching & Testing: Help clients rediscover personal career goals and work towards achieving them over time
- Life Skills Training & Support: Teaching essential life skills from budgeting and saving to engaging in self-care
